Certificate FAQ

 

NOTE: We cannot issue a CE certificate for partial attendance of a course. If you attended a live course, Zoom attendance must relay that you attended the entire training. If you took an asynchronous/self-paced webinar, you must complete and pass the quiz before being eligible to receive an End of Course Evaluation and Certificate.

 

I never received my certificate!
Did you complete an online or in-person training and not receive a certificate? Please know that you must complete an End of Course evaluation to receive a certificate per CE rules. Did you complete an End of Course Evaluation for the training?

  • If not: Be sure to check your spam or junk folder for any missed emails. We typically send the End of Course evaluation link to participants who have completed a live training within 48 business hours. Once your End of Course Evaluation is submitted, your certificate is downloadable at the end of the evaluation. If you are unable to find any of our previous post-training emails, please email admin@micenterforchange.com
  • If you completed the End-of-Course Evaluation but did not receive your certificate, please complete this form. We’ll review your course attendance and completion details and re-issue your certificate accordingly.

I lost my certificate and need another copy!
To request an additional copy of your certificate, please complete the form below. Note that a $25 processing fee applies for re-issuing lost certificates.
