I never received my certificate!

Did you complete one of our online or in-person trainings and submit the end-of-course evaluation?

If not: Be sure to check your spam or junk folder for any missed emails. We typically send follow-up emails within 48 business hours. Once your evaluation is submitted, your certificate will be issued. If you are unable to find any of our previous post-training emails please email admin@micenterforchange.com
If yes: Please complete the form below, and we’ll be happy to issue your certificate.

Please note: This form is only for participants who completed the end-of-course evaluation but did not receive their certificate. If you are requesting a copy, a small processing fee applies for re-issuance.
